You can do this by using FreePDF to print an existing PDF file to a new
one.
It can be got from www.shbox.de
FreePDF has an 'Add to Next PDF' option which allows you to merge two
(or more) 'printings'. You can split an existing PDF file by simply
printing (for example) 'Page 2 to Page 5' to a new file (but not,
unfortunately, 'Pages 2,4 and 5').
