I don't use outlook but...

All of the email clients I've worked on already save attachments to a
default subdirectory. Have you identified what the default is for your
outlook?

If you can do that, then you can either

1) go to that sub-dir and transfer to your /pix folder. Then have
picasa index it or
2) point Picasa2 at the sub-dir and let it index it

Personally, I prefer to have my picture files in a certain physical
drive and directory structure. So, I use #1.
